This Change of Address form is provided for Dorset licence holders.

What you need to complete this form:

• Certificate numbers for all currently held firearm licensing certificates.
• Your previous home address.
• Your current home address.
• If you have registered at a new GP surgery, the address of your surgery and the name of your allocated GP.
• Photographs of your security arrangements to include;
  • Cabinet with no gaps or leverage points from both sides (Max of 2 Images)
  • Image of the top of the view of cabinet. (Max of 1 image)
  • Cabinet open with no guns within and the bolt fixings visible. (Max of 1 image)
  • Wide view of the room the cabinet is situated in. (Max of 1 image)

Details of Each person (including children) with access to your address please provide the following information;

1. Title
2. First Name
3. Surname
4. Date of Birth
5. Place of Birth
